We want the day of your surgery to be a seamless experience, where you can simply walk in, check-in at our front desk and be shown to your room rather than fill out pages of paperwork.
Registration is designed to be quick and easy and completing the form should take you 20-25 minutes. Please have your insurance information handy.
When you arrive at our center, we ask that you check-in at the front desk to verify personal, companion and insurance information.
Our Day of Surgery Guide will give you all the details on how you can prepare for your surgery.
Offering easy to understand recovery instructions for you and your family is our priority. Read through our guide for more information.
Be sure to arrange a ride option for arriving home safely after your procedure. Read through our After-Surgery Guide for more important information.
We continue to serve patients and provide them with much needed, high quality, lower cost elective surgeries in a healthcare setting where we don’t treat COVID-19 patients. We focus on safety through expanded policies and protocols in accordance with proven national health and safety guidelines.
Your visit may take longer due to our expanded safety measures, some of which may change over time. For further questions please contact us at 239-333-2560.
Colonial Outpatient Surgery Center is committed to maintaining your satisfaction with our facility and resolving any concerns you may have.
We are committed to providing quality healthcare. Proof of this commitment is our accreditation by the Accreditation Association for Ambulatory Health Care (AAAHC). We strive to provide the best care possible by meeting and exceeding AAAHC standards for high-quality healthcare.
More detail can be found on our Transparency in Healthcare page.
There are many quality measures and statistics related to ASC’s through the AHCA (Agency For Healthcare Administration). You can access via the link AHCA has provided and we have made available to you below.
The Agency for Health Care Administration (AHCA) pricing website below provides information on payments made to the facility for defined service bundles and procedures.
According to AHCA, “the service bundle information is a non-personalized estimate of costs that may be incurred by the patient for anticipated services and…actual costs will be based on services actually provided to the patient.”
We are committed to ensuring that our website and mobile applications are accessible to individuals with disabilities. If you need assistance using our website or mobile application, or assistance with a document on the website or application, we can help you.
